Home Medical Care


Caregiving Welfare Association (CWA) aims to provide quality care that promotes the elderly’s well-being at home, and reduce the need for hospital re-admission or being institutionalized, thereby meeting the health and social care needs of the elderly.

How are our services different?
Our professional caregivers can offer you the following value-added
services:
• Our caregivers will be trained in CPR & First Aid
• Any changes in behaviour/condition detected by our caregivers will
be timely referred to the hospital
• Memos of the seniors’ habits, e.g. what may have led to change, will
be provided to speed up assessment by hospital

Services catered for our clients
Nursing Services
• Simple wound dressing procedures
• Maintenance of catheters and drainage tubes
• Post-surgical procedures

Medical Services
• Monitoring Medication Compliance
• Monitoring blood pressure, hypo count etc.

Medical Escort Services
• Bring clients for medical appointments
• Assist in navigating the hospital systems

Home Rehabilitation
 Conduct simple physical exercises prescribed by Physiotherapists
• Advise on the usage of assistive devices
e.g. walking frames etc.

Who should apply?
Any homebound person who is unable to attend medical
appointments on his/her own or have no suitable caregiver to
accompany him/her there may apply for the following homecare
services:

Medical Services
Medical Escort Services

Any frail or disabled senior who has been assessed and need
assistance in ADL or iADL may apply for Home Personal Care.

Home Medical Care at Caregiving Welfare Association

Through these services, frail and homebound elderly will be assessed, and individual care plan would be tailored for each patient. The home medical and basic therapy services, which include:

  • Wound Management
  • Vacuum-Assisted Closure (VAC) Therapy*
  • Nasogastric Tube Changing
  • Continence Care**
  • Stoma Care
  • Medication Management
  • Caregiver Training
  • Counselling
  • Others: Mucous, suction, injection, and basic physical therapy.
*subjected to availability of the nurse.
**urinary catheterisation (female)

Service Fee


Those who are eligible through means testing will be charged based on a sliding scale.
