Role: Overseeing care services and focus on quality care

The Community Nurse/ Care Coordinator is responsible for planning & coordinating, the patient care services and provides supervision to our group of Community Caregivers. It also includes providing quality care to clients under the Home Based Care.
The role is also responsible for managing and providing direct patient care through care management, assessment, providing home personal care services, nursing procedures and evaluation of nursing care to meet the needs of the patients, including educating patients & families and caregiver training.
Duties/Responsibilities
1) Home Based Care
a) Patient Care
- Assess, plan, implement and evaluate patients’ needs
- Provide management and supervision to a team of Community Caregivers (CCGs)
- Development, implementation and review of clients’ care plans
- Facilitate patient education
- Refer patients for further care or services, when necessary
- Serve as a clinical resource person for CWA and CCGs
- Conduct home visits
2) Administration and Reports
- Manage resources needed to support CWA’s Home Based Nursing operations.
- Handle administration work and reports
- Participate in research/ projects as assigned

- Assist and support all ad-hoc events organised by CWA and/or third party organisers when assigned, and assist in any other events of the CWA which may be held in the evenings and possibly on Saturdays, Sundays and Public Holidays.

Requirements/Qualifications
- Registered Nurse with the Singapore Nursing Board
- Degree or Diploma in Nursing
- Fresh graduates are welcome to apply, as long as you are passionate in the Community Sector
- Have passion in caring for elderly in the community sectors
- Comfortable entering clients’ homes for assessment
