Caring for a senior loved one can feel overwhelming. That’s why CWA created CareJourney — a structured and guided pathway to support caregivers at every stage: whether you’re just starting, actively providing care, or adjusting after your caregiving journey.
Launching in November 2025, the pilot will begin with caregivers supporting senior loved ones living with stroke and dementia, conditions that often bring complex challenges for families.
Through CareJourney, you’ll be guided, connected, and supported — so you never walk this path alone.

What We Offer
For New Caregivers
- Step-by-step guidance to get started with confidence
- Practical knowledge to manage daily routines and care needs
- Resources to help you understand common conditions like stroke and dementia
For Active Caregivers
- Skills training and workshops to make caregiving easier
- Emotional support and peer groups to reduce stress
- Direct connections to trusted professionals and services with continuous support
For Former Caregivers
- Opportunities to share your experiences and mentor new caregivers
- Continued involvement in the caregiving community
- Support in transitioning to life after caregiving
Eligibility Criteria
You can join if:
- You live with and care for a senior (not a helper)
- The senior is 60 years or older
- The senior has stroke or dementia
- The condition will last at least 6 months
- The senior has stroke or dementia
- The senior needs help with 3 or more daily activities

Who can join CareJourney?
CareJourney is for family caregivers who live with and care for seniors aged 60 and above, diagnosed with stroke or dementia, and who need help with at least 3 daily activities.
How is CareJourney different from other programmes?
With CareJourney, you don’t need to re-explain your needs to every provider. We link you directly to a trusted network of partners, so you get faster help and smoother support as your loved one’s needs change.
What kind of support will I receive?
For new caregivers:
You’ll receive step-by-step guidance, practical knowledge, and resources to help you understand your loved one’s condition and build confidence in daily care.For active caregivers:
We provide training, practical tools, and emotional support. You’ll also be linked directly to trusted professionals and services, so you get faster help without having to re-explain your situation every time.For former caregivers:
We offer opportunities to stay engaged in the caregiving community, share your experiences to support others, and guidance on transitioning into life after caregiving.
