What is Case Management?
Case Management is a collaborative process that:
Identifies presenting issues and unmet needs
Develops a care plan tailored to the client’s situation
Coordinates with relevant agencies and programmes for timely support
Adapts care plans as the client’s circumstances evolve
Our approach considers not just the individual, but their family dynamics, social setting, and overall environment.
Need Support or Want to Refer Someone?
Our Case Management Service offers personalised psychosocial and emotional support to seniors and their caregivers through our team of qualified social workers. Services may include supportive counselling, clinical interventions, and support group therapy.
Support is delivered based on assessed needs and may take place at your home, via tele-consultation, or at our headquarters.
Referrals are accepted from social service agencies, medical institutions, and community partners. If you would like to refer a client, please complete our referral form here.
